Encountered this problem on a client site. Main plugins are qTranslate 2.5.28 and Advanced Custom Fields 3.1.6 running on WP 3.3.1. Michaels solution works perfectly, but there is one more thing to add. Just to keep things simple for the "not-so-advanced" user. Removing the tabs for "Visual Editor" and "HTML Editor". To remove the Visual Editor the normal way will of course render qTranslate crippled. Instead edit wp-admin.css and add

Simple solution to keep things on track.
